Happens to me as well from time to time. 

Perhaps refreshing the game after every attack might fix it?

I've only ever had it a few times, but it's rare

It happened too. It's probably a bug. I would like you to fix it for the update.

I'm pretty sure I know why it happens, and it likely has to do with switching servers.

your best bet would be to just restart the game before you begin doing your attacks. that's what I've been doing ever since I last had it happen to me and since then it hasn't shown up.
