What does it mean for an item to be premium?

a premium item is one which's rarity starts at legendary (epic items capable of transforming into legendary aren't premium and so on)

In that being said, I pretty much have to get lucky but it isn't locked to those who don't spend real money on the game?

it's entirely possible to obtain premiums while being f2p, yes

yes, many top players are real free to play all parts.
